• Determines and documents through evaluation if the client's needs the level of Plan Of care.
  • If the client's needs exceed the personal care services, the agency shall make a referral to a licensed health care professional or an appropriate alternative service.

  • Demonstrate efficient teamwork with the staff.
  • Coordinate care for the client. *
  • Understand and perform the admission process plan of care. *
  • Coordinates services and schedules client visits.
  • Supervise field staff as indicated. *
  • Perform the initial home care visit and re-evaluate client needs and progress when needed. *
  • Notify physician of change in the client’s condition. *
  • Coordinate and monitor all client care and services. *
  • Complete paperwork timely and efficiently. *
  • Maintain client records according to policy and procedure. *
  • Follow infection control policy in and out of the office. *
  • Provide quality of care to clients in their homes according to policy and procedures. *
  • Participate in the on-call schedule. *
  • Participate in and support quality improvement programs.
  • Skilled nursing service(s) must be reasonable and necessary for the treatment of the illness or injury, that is, the services must be consistent with the unique nature and severity of the member's illness or injury, his or her particular medical needs, and accepted standards of medical and nursing practice, without regard to whether the illness or injury is acute, chronic, terminal, or expected to last a long time.
  • Skilled home health nursing care is the provision of intermittent skilled services to a member in the home for the purpose of restoring and maintaining his or her maximal level of function and health.  These services are rendered in lieu of hospitalization, confinement in an extended care facility, or going outside of the home for the service.
